The general history of the health topic a) When did it become a national concern? b) What are the national morbidity and mortality rates for this health topic? c) How has it change over the past few decades at the national level? Give morbidity and mortality rates for the last few decades. A chart would be very helpful. 3. The determinants of the health issue: What are the contributory causes? What make people at-risk for it? 4. The efforts that have been taken to reduce the incidence rate at the national level. What specific national programs/agencies/nonprofits are focused on this health topic? County specific information 1. Describe the burden of disease at the county level including Mortality/morbidity rates Financial costs to the county (if you cannot locate this, the financial cost to an individual with this health condition) Impact on the quality of life including Disability-Adjusted Life Years (DALY) & Years of Potential Life Lost (YPLL).
