One of the most important areas of nursing practice that has improved patient outcomes is using two patient identifiers when passing medications. There have been so many problems in the past that has since been lessened by the nurse having to use at least two patient identifiers before passing medication to that patient. According to (AHC MEDIA, 2018, p. 1), “Medication errors are one of the most commonly cited reasons for adverse events”. It is very important that we practice the six rights of medication administration when dealing with patient’s safety. The six rights include, right client, right route, right drug, right dose and right time.

The second area of nursing practice that has improved patient outcomes is to identify the patient’s barriers to care. Some times this can be a cultural barrier, language barrier or knowledge-based barrier. Once the barrier is identified, the nurse can work with the patient and introduce a more patient friendly version of the care plan to better fit that patient’s needs. Other times, it is actually due to the nurse not having adequate time to explain the discharge instructions leaving the patient to fend for him or herself. Reported by (Nursing Economic$, 2004), there has been a lack of adequate written and verbal patient education during discharge instructions due to nurses being time pressured to move on to the next patient or duty at hand.
